Potential Tattoo Form

Please copy/paste this form and fill it out if you are interested in getting tattooed. All emails must be sent to [email protected] under the subject "TATTOO REQUEST FORM". Anyone not following these instructions will be skipped over, so it's important to fill everything out to the best of your knowledge! 

1. What are you trying to get tattooed? (design ideas and/or reference photos are preferable!)
2. Where are you trying to place this tattoo? Is the area surrounded by tattoos already? Is this a cover up?
3. What is your budget, if you have one? (This will help determine if the tattoo you want is doable for your budget.)
4. Do you live less than an hour away? If no, where are you located and how far is it from Jacksonville, NC?
5. When are you trying to get tattooed? Do you have a specific date/days that you are inquiring about?
6. Is this your first tattoo?
7. Are you a return client?
8. Please provide your first and last name, phone number, and date of birth.
